A photocopy of the Foreigner Physical Examination Form

The form is issued by the Chinese quarantine authority. The physical examinations must cover all the items listed in the Foreigner Physical Examination Form. Incomplete records or those without the attending physician’s signature, the official stamp of the hospital, or a stamped photograph of the applicants are invalid. Please select the appropriate time to take a physical examination, as the result is valid for only 6 months.

Non-criminal record report

The certificate of Non-Criminal Record should be issued by the local public security authority. It should be issued within 6 months prior to the submission date of the application.

 A notarized highest academic diploma or certificate

Perspective diploma recipients must submit official proof of student status in their current school.

Academic transcript

Copies of academic records from the first to the last semester during the undergraduate, master’s (if any), and doctoral programs (if any). These trans shall be issued and sealed by the attended school or university’s academic affairs office, graduate school, or student management department. They can be in Chinese or English and should be attached with notarized English translations if they are in foreign languages other than English.

Digital application photo

The photo should be taken within 6 months prior to the submission date of the application;

Background: white or off-white;

Size: 35 mm width * 45 mm height;

Head size: 2/3 of the photo;

Image pixel dimensions: no less than 320*240 pixels;

Image size: 100-500KB in JPG file format

HSK5 or HSK4 Certificate

Valid visa and resident permit

Provide a copy of valid visa and resident permit if the applicant is already in China.
